Home Care Specialist Role

Schedule and Availability:

This role requires working Monday to Friday, with shifts starting at 07:00 AM and ending at 04:00 PM.

Key Responsibilities:

  1. Develop and maintain a rostering system for support worker allocation, ensuring efficient use of resources and effective care delivery.
  2. Manage client visit schedules and assignments based on availability, skills, location, and preferences, minimizing scheduling conflicts and coverage gaps.
  3. Collaborate with participants, families, and healthcare professionals to establish and update care goals, reviewing and adjusting care plans for effectiveness.
  4. Provide ongoing support and performance evaluations to care providers, building strong relationships with participants and their families as primary contacts for concerns.
  5. Maintain accurate records of support worker availability, client preferences, and visit schedules, generating reports on rostering activities, including service utilization and client satisfaction.

Requirements:

  1. Healthcare background and clinical understanding are essential for this role.
  2. Tertiary qualification in Health or a related field, with relevant care management experience, is required.
  3. Previous experience in rostering or scheduling, ideally in healthcare or home care, is highly valued.
  4. Strong knowledge of home care regulations, industry standards, and best practices is necessary for this position.
