Patient Care Coordinator

The Patient Authorization Coordinator plays a pivotal role in our organization, interacting closely with various stakeholders. This individual works alongside clinical sales specialists, management team leaders, physicians, nurses, and office staff to ensure seamless coordination of care for incoming patients.
The Intake Coordinator provides critical support under the direction of the Pharmacy Supervisor/Intake Team Lead, contributing significantly to customer satisfaction and overall business success.
Responsibilities- Receives referrals from pharmacists after review, including insurance information, statement of medical necessity, prescription, and pertinent clinical documentation necessary for authorization.
- Assists in obtaining required information under the direction of the Pharmacy Supervisor/Intake Team Lead, utilizing the Docutrack system for tracking and monitoring appropriate designation of received information.
- Enters patient information into Premier View for designated territorial new starts.
- Assists with incoming referrals for other territories as needed.
- Enters territorial requests for follow-up assistance on the Request log as appropriate.
- Consults with clinical staff regarding clinical review, contacting physician offices as directed and requesting prescriptions as needed to process referrals.
- Monitors formulary status of incoming referrals and assists with initial follow-up according to insurance plan dictates.
- Assesses new start referrals for patterns and trends related to completeness of referral and insurance/formulary coverage.
- Informs families of status if delay at or beyond 72 hours occurs with an incoming new referral.
- Forwards information to case managers for processing upon completion of data entry.
- Consistently updates notes in the system and logs to reflect current patient status.
- Notifies parties of file inactivity and notifies all relevant parties when service is unable to be provided based on current coverage or contracting limitations.
- Works closely with Sales/Clinical staff and Case Managers to communicate referral status.
- Makes initial contact with DME patients.
- Performs other related duties as assigned and required.
- Strong verbal and written communication skills are essential.
- Must possess basic knowledge of office operations and be able to perform multiple tasks simultaneously.
- Ability to work under pressure and prioritize tasks during periods of high workload is necessary.
- Must represent the company in a professional manner.
- Understanding of HIPAA compliance principles, maintaining confidentiality and security of patient information.
- Able to perform duties without posing a direct threat to self or others' health or safety.
