Work from home as a Care Coordination Specialist to support seniors receive the care and attention they deserve.

About the Role

This remote, work-from-home position involves coordinating caregiver schedules and ensuring smooth operations for clients and caregivers. You will build and maintain up-to-date caregiver schedules across multiple counties and respond quickly when a shift is canceled or a caregiver does not clock in. Additionally, you will answer calls from clients and caregivers using the VoIP system, update records and case notes in WellSky, and work with the operations team to flag issues and maintain staffing smoothness. Furthermore, you will conduct regular check-ins and evaluations with caregivers, generate weekly reports, and keep leadership informed.

Key Responsibilities
  • Build and maintain up-to-date caregiver schedules across multiple counties

Requirements
  • You have experience in scheduling or can perform well in scheduling (bonus points for healthcare or home care experience)
  • You are calm under pressure and naturally organized
  • You enjoy solving problems on the fly
  • You are comfortable with software systems and remote communication
  • You build strong relationships with stakeholders and are a trusted partner
  • You take initiative while being a team player
  • You are willing to work in a US time zone schedule
