Chronic Care Specialist

We are seeking a dedicated and compassionate professional to join our team as a Chronic Care Specialist.

In this role, you will be a vital lifeline connecting with patients who have chronic conditions, guiding them in managing their health, and making a real impact on their well-being.

About the Job:

  • Connect with patients: Make regular outreach calls to provide support and guidance on managing chronic conditions.
  • Personalized care plans: Collaborate with patients and healthcare providers to design and update individualized care strategies.
  • Early intervention: Monitor symptoms and identify risks to prevent complications.
  • Coordinate care: Work seamlessly with primary care providers, specialists, and community resources.
  • Patient education: Equip patients and caregivers with knowledge on disease management, medication adherence, and healthy habits.
  • Document with precision: Keep accurate and timely patient records.
  • Stay compliant: Follow CMS guidelines for chronic care management while promoting preventive care and lifestyle improvements.

Requirements:

  • Valid RN license.
  • Minimum 1 year of clinical nursing experience (chronic care, case management, or primary care experience is a plus).
  • Strong knowledge of chronic conditions such as diabetes, COPD, hypertension, and heart failure.
  • Excellent communication, critical thinking, and organizational skills.
  • Comfortable with electronic health record systems and virtual care tools.
  • Able to work independently while thriving in a collaborative team setting.

Bonus Points:

  • Familiarity with Medicare Chronic Care Management documentation and compliance.
  • Experience in motivational interviewing or other patient engagement methods.
