Staff4Me is seeking a dedicated and proactive Case Coordinator to join our team. The Case Coordinator will be responsible for overseeing the care and services provided to clients. This position is integral to ensuring clients receive the appropriate support and resources throughout their care journey. You will collaborate closely with a multidisciplinary team to develop care plans, monitor client progress, and facilitate communication between clients and service providers.

Responsibilities
  • Coordinate the delivery of services and support based on individual client needs.
  • Conduct assessments to determine the necessary resources and services for each client.
  • Develop and implement personalized care plans in collaboration with clients and their families.
  • Monitor clients\' progress and make adjustments to care plans as needed.
  • Serve as the main point of contact for clients and their families, addressing questions and concerns.
  • Facilitate communication between clients, healthcare providers, and community resources.
  • Document and maintain accurate records of client interactions and care plans.
  • Participate in case reviews and interdisciplinary team meetings to discuss client progress.
  • Stay informed about available community resources and support services.
  • Ensure compliance with regulatory requirements and organizational policies.
Qualifications
  • Bachelor\'s degree in Social Work, Nursing, or a related field.
  • Minimum of 2 years of experience in case management or coordination within a healthcare or social services setting.
  • Strong assessment and analytical skills.
  • Excellent communication and interpersonal skills.
  • Ability to work collaboratively in a multidisciplinary team environment.
  • Knowledge of community resources and support services available to clients.
  • Strong organizational and time-management skills.
  • Proficiency in using electronic medical records and case management software.
  • Compassionate and patient-centered approach to care.
  • Ability to handle sensitive information with confidentiality and professionalism.
