About the client: The client is a leading, multi-location outpatient physical therapy practice in the U.S. specializing in orthopedics, sports, and manual therapy. They combine hands-on treatment with individualized exercise programs to help patients recover from injuries, manage pain, and return to active lifestyles. • Real-time EHR Documentation: Accurately document patient encounters directly into the EHR during or immediately after visits; capture relevant history, objective findings, assessments, treatment plans, and outcomes in clear, structured notes; use clinic templates and standards to ensure notes are billable, compliant, and ready for provider sign-off.
• Clinical Correspondence & Records: Prepare and edit patient notes, visit summaries, progress reports, and letters to referring physicians; maintain consistency of records across encounters and ensure timely chart updates; organize and track outstanding documentation for provider review.
• Billing & Insurance Support: Assist with documentation needed for medical necessity, insurance authorization, and claims support; enter or verify coding-related details (ICD/CPT as directed) and resolve documentation gaps that impact billing; coordinate with admin staff to ensure clean claims and reduce rework.
• Data Quality, Timeliness & Compliance: Perform accuracy checks for patient identifiers, visit details, and plan of care before submission; adhere to HIPAA and clinic privacy/security protocols; meet same-day or agreed-upon turnaround times for chart completion.
• Collaboration & Communication: Communicate clearly with U.S.-based providers and admin teams during working hours; flag urgent issues, clarify missing information, and respond promptly to requests; support end-of-day reconciliations and carry-forward tasks to keep workflows current.

• Proven experience as a medical scribe, medical virtual assistant, or healthcare documentation specialist
• Strong written and spoken English communication skills
• Typing speed of at least 45 WPM with high accuracy
• Working knowledge of basic medical terminology (healthcare/clinical context)
• Proficiency with EHR documentation and Microsoft Office (Word, Excel, Outlook), with strict adherence to HIPAA
• Can commit to working 40 hrs/week, Monday–Friday, 11:00 a.m.–8:00 p.m. Eastern Time
• WFH Set-Up:
  • Computer with at least 8GB RAM, an Intel i5 core processor/AMD Ryzen 5 Processor and up.
  • Internet speed of at least 40MBPS
  • Headset with an extended mic that has noise cancellation and a webcam
  • Back-up computer and internet connection
  • Quiet, dedicated workspace at home
