Job Description As a Care Coordinator , you will ensure seamless service delivery for clients by managing schedules, supporting carers, and providing exceptional communication and care coordination. Visit Free 24/7 access to our office gyms (Ortigas and Makati) with a free physical fitness trainer Exclusive Emapta Lifestyle perks (hotel and restaurant discounts, and more) Unlimited opportunities for employee referral incentives across the organization Standard government and Emapta benefits Total of 20 annual leaves to be used on your own discretion (including 5 credits convertible to cash) Fun engagement activities for employees Mentorship and exposure to global leaders and teams Career growth opportunities Diverse and supportive work environment The Qualifications We Seek 1 to 3 years of experience Experience in a similar role, preferably within the Disability or Aged Care sectors Ability to work independently and achieve results in a fast-paced environment Strong problem-solving and multitasking skills Familiarity with client management systems Proficient in Microsoft Word, Excel, SharePoint, and Outlook Understanding of the National Disability Insurance Scheme (NDIS) Your Daily Tasks Facilitate the whole service delivery for clients Manage the roster of carers and support carer engagement to ensure optimal use of their availability Assist carers with accessing and using the electronic rostering system Respond to carer enquiries Be the first point of contact for client enquiries, incidents, complaints, and feedback Confirm and manage bookings with clients and service providers Maintain regular contact to ensure client satisfaction and wellbeing Provide progress reports to families, carers, and stakeholders Monitor client funding and advise on the best use of available resources Follow up on client renewals, service holds, and opportunities for re-engagement Receive and manage incoming calls and service requests Participate in the after-hours roster as required Perform other duties as required About the Client Empowering Care, Enriching Lives Our client is an Australian healthcare organization committed to delivering personalized and high-quality care to individuals with disabilities and aged care needs.
