AtHealth Monitoring1, we’re changing the narrative by providing in-person cardiovascular care directly to patients in Nursing Homes and Assisted Living Facilities. Our services include: In-person visits for chronic disease care Collaborative care, partnering with PCPs, cardiologists, and facility teams to improve patient’s outcomes. Remote heart and blood pressure monitoring What You’ll Do – Your Role Conduct in-person, preventive cardiology and chronic disease-focused visits in Assisted Living and Skilled Nursing settings Monitor patients’ chronic cardiac conditions daily, using remote technology Collaborate with primary care providers, other cardiologists, and our internal team. Document clinical notes and treatment plans with attention to detail Educate patients, families and staff on heart health, prevention, and wellness Contribute to a culture of excellence and patient-centered care What You Bring – The Ideal Candidate Active Nurse Practitioner License (Florida) Strong background or interest in Cardiology or Geriatrics experience, working in long-term care settings preferred Proactive care Integrity, autonomy, and respect Excellent communication and collaboration skills Tech‑savvy and comfortable using remote monitoring tools Organized, detail‑oriented, and passionate about prevention Willingness to travel locally between facilities Our Purpose To make a real difference in the lives of seniors by preventing the worsening of chronic conditions.
