Join to apply for the FPS Case Manager Mission role at DCCCA 6 days ago Be among the first 25 applicants Responsibilities Provides for the safety, stability and well‑being of children and families during the referral period Delivers culturally competent, strengths‑based case management services to children and families consistent with evidence‑based practice models Decides on guidelines provided by the supervisor, family preservation standard procedures and policies, evidence‑based practice model and DCCCA organizational policies Meets DCF timelines to connect with the family within 24 hours of referral; facilitates initial family meeting within 48 hours of referral; completes assessment‑based case plan within 20 days of referral; and case closure within 3‑6 months of referral Engages and works in partnership with families to complete assessments and develop case plan goals and objectives Meets with the family in the home weekly, monitoring the family's progress toward their case plan goals; and meets monthly with each child individually to assess child's safety Assesses each child's safety with every family contact and reports all suspected child abuse/neglect; monitors family progress and service provision using assessment tools, and updates family case plan based on those assessments Provides weekly home‑based family skills training which may include: nutrition, hygiene, household and time management, home and child safety, basic parenting, transportation, problem solving and communication to assist parents in providing a safe, nurturing environment for children in their care Works closely with the Family Support Worker to provide services for the family and make decisions about child well‑being Prioritizes identified problems that require resolution, establishes goals and objectives, utilizes available resources, and arrives at decisions appropriate to the family Completes and analyzes initial and ongoing family assessments related to safety, stability and well‑being, including mental health, substance abuse, education, emotional, social and physical well‑being, parenting and parent‑child relationships Utilizes assessments to develop the case plan in partnership with the Family Support worker/Family Mentor, child/ren and family; makes community referrals as necessary Completely and submits timely court reports; appears and testifies in court as required Collects drug screens as necessary Documents case information, including court reports, emails, and activities weekly Responds promptly to family crisis situations, documents incidents and is available 24/7 to clients Job may require the employee to be able to regularly drive to various DCCCA locations and other business locations throughout the day or night as required Promotes and practices teamwork in all activities Fosters and maintains professional relationships with families, community partners and peers Participates in professional development that is relevant Seniority Level Entry level Employment Type Full‑time Job Function Other Industries Non‑profit Organizations Education, Experience, and Licensure Bachelor's degree in human services, required Prior work in behavioral health or children and family services, required Experience working with substance use, preferred Must possess a valid Driver's License, have in‑force automobile liability insurance coverage, and maintain a Motor Vehicle Record (MVR) that meets organization's insurability standard as defined by DCCCA policy.
