Job Title: Patient Care Coordinator

Join our team as a Patient Care Coordinator and play a vital role in delivering exceptional patient care.

  • Key Responsibilities:
    • Assist patients with prescription, visit follow-up, non-medical questions, and test result inquiries.
    • Continuously review the in-basket to triage all unread messages and respond accordingly.
    • Communicate with patients via telephone for critical issues and with local sites via telephone or email as needed.
    • Perform chart reviews and respond directly to patients through Epic MyChart.
    • Route messages requiring further clinical assistance to the appropriate pool or provider.
    • Process medication refill requests and route to providers.
    • Document patient interactions and care in the Epic EHR through the MyChart encounter.
    • Review and complete tasks as necessary or route to local site administrative pool.
  • Qualifications:
    • Must be a US Registered Nurse – New York Licensed.
    • Graduate of BS Nursing with active Philippines Registered Nurse.
    • At least 2 years of clinical or hospital experience.
    • Able to work night shifts.
    • Demonstrate critical and independent thinking.
    • Comfortable providing patient education on chronic conditions.
    • Excellent communication, interpersonal, and written skills.
    • Strong computer skills, including Microsoft Office suite and Electronic Medical Records.
    • Excellent customer service skills.
