Position Overview:
The Breast Cancer Patient Navigator will provide exceptional, coordinated and patient-centred care that ensures timely access to specialist appointments and procedures. This Patient Navigator follows evidence-based pathways to ensure that patients with any abnormal finding in their breast have quick access to the required diagnostic procedures. The Patient Navigator will remain closely connected with the patient and the primary care provider to ensure that both are informed about every step of the care plan. Navigating patients during a health crisis is one of the key roles of this position.
Reporting to the Oncology Manager, the Patient Navigator will support the patient′s journey through the diagnostic phase of care and will work collaboratively with the interprofessional team. The Patient Navigator will apply his/her knowledge of the health care system and the cancer system to successfully advocate for the patient/family. The Patient Navigator will be accountable for facilitating the necessary next steps along the care trajectory. Although this position will primarily practice within the breast site group, the navigator will be part of a nursing team supporting other oncology patient navigators and will be expected to provide support as required.

Requirements:

• Bachelor′s Degree in Nursing and current registration with the College of Nurses of Ontario (in good standing),

• Current BCLS

• Completion of courses with the deSouza Institute – including at minimum Patient Navigation, and Foundations in Oncology Nursing Practice

• Evidence of continuing education in the area of oncology nursing and care of the patient with breast cancer

• Evidence of advanced assessment skills and demonstrated clinical knowledge in the area of breast health concerns, breast cancer treatments and psychosocial impact of disease

• Demonstrated ability to work collaboratively within an interprofessional care team

• Experience with the development and implementation of effective education programs targeted at patients and families,

• Proven ability to effectively and efficiently coordinate, implement and monitor the plan of care for each patient

• Excellent communication skills with proven ability to collaborate with patients, caregivers, and members of the health care team both within the hospital and the community

• Demonstrated telephone assessment and triage skills

• High degree of professionalism and initiative, with the ability to function independently in a position with limited immediate supervision

• Proven good attendance with the capability of maintaining the same standard.
