Qualifications & experience

  • Bachelor of Science in Pharmacy Graduate.
  • Completion of a Clinical Pharmacy Training Program from an accredited institution offering the Clinical Pharmacy Certification Program.
  • Graduation from an educational institution with a continuing Clinical Pharmacy Program.
  • Proficiency in Microsoft Word and Excel.

Tasks & responsibilities

  • Analyzes the present medication regimen by reviewing medical and medication history of the patient.
  • Monitors appropriate use of medication ensuring that the standards in medication ordering and administration are met. (Dosage, route, frequency, computation, dilution, etc.)
  • Attend to the diagnosis, treatment, and care of patients during assigned clinic hours.
  • Conduct examinations and provide evaluations on the health progress of patients.
  • Coordinate activities with the section chief and adhere to all prescribed policies and guidelines set forth by the department.
  • Provides reports on activities, interventions, and results from units being monitored and files relevant documents such as but not limited to medication error forms, adverse drug reaction forms, drug therapy monitoring and assessment forms etc.
  • Adheres to the confidentiality of documents/records of the patient's information, taking into account the governing policies of the Hospital in relation to the Data Privacy Act of the country and to the Hospital's implemented plans, programs, and budgets.
  • Shall attend relevant trainings and seminars and various educational tools for professional growth.
  • 1 free meal a day
