I.
JOB
SUMMARY:

Hospital
pharmacists' duties include administrative duties, clinical service patient
care, drug distribution, drug control, ensuring medication safety, and
activities for quality and performance improvement. Hospital Pharmacist serves
patients by preparing medications, giving pharmacological information to the multidisciplinary healthcare team, and monitoring patient drug therapies.

II.
DUTIES AND
RESPONSIBILITIES:

v Dispensing Pharmacist

Ø Fills and dispenses
all requisitions from different nursing care units and special areas.

Ø Checks displays on the
shelves and composes requisitions of medicines from the main pharmacy.

Ø Answers telephone
inquiries.

Ø Responsible for the
cleanliness of the shelves assigned to her/him and monitors the expiry dates of
drugs on those shelves in accordance with the pull-out policy.

Ø Does physical
inventory on the assigned shelves every first week of the month. All expiry
items will be withdrawn on the 1st day of the month for sealed items
and at the end of the month of expiry for loose medicines.

Ø Receives telephone
manual posting of medication from OR mini-pharmacy; sends a reprint of charges
to OR information desk nurse and secretary for out-patients under cash basis
accounts.

Ø May be assigned in the
Pharmacy Office with the following duties and responsibilities:

i. Purchases non-formulary drugs

ii. Prepares all supply requisitions from the different units/departments in ARC Hospitals.

Ø Performother necessary duties that maybe assigned by the Senior Pharmacist
or Chief Pharmacist.

v Receiving Pharmacist

Ø Receives
physician's drug order sheet faxed from the nurse station and checks for any
erroneous, violative, and impossible physician's order.

Ø Clarifies
any vague physician orders by informing the station's charge nurse.

Ø Record and
file those orders designating one medication profile for each patient.

Ø Coordinates
with resident physicians, charge nurses, and staff nurses for any inquiries or
problems that may arise.

Ø Verifies
medication profile versus the medications requested by the nurses before
releasing the drugs.

Ø Does
profile review of patients for any Large Volume Parenteral medication that will be due within their shift.

Ø Makes
sticker tickets and charges the patient with materials to be used in compounding
the medication.

Ø Answers telephone drug inquiries.

Ø May be assigned in the Pharmacy Office with the following duties and
responsibilities:

a) Purchases

Ø Performother necessary duties that maybe assigned by the Senior
Pharmacist or Chief Pharmacist.

v Compounding Pharmacist

Ø Receives
compounding order from the Receiving Pharmacist, counter-checks the Physician's
Drug Order Sheet, and creates a Medication Profile for each patient's Large
Volume Parenterals (Incorporation/Infusions) and Controlled Drugs.

Ø Observes proper Personal
Protective Equipment (PPE) before compounding.

Ø Compounds
controlled intravenous drugs and medication that needs to be incorporated or
infused in a large volume parenteral preparation under aseptic condition.

Ø Attach
sticker tickets and all necessary auxiliary labels to the prepared medication
affixing his signature on the drug label and seal for dispensing.

Ø Disinfects
the clean bench with 70% alcohol and UV light for 15-30 minutes at the end of the
shift.

Ø Disinfects
the clean bench with Solarsept Solution scheduled every other Saturday for the total eradication of potentially disease-causing microorganisms.

Ø Responsible for maintaining the cleanliness of his working area and the
availability of the materials used in compounding medications.

Ø Perform other necessary duties that may be assigned by the Senior
Pharmacist or Chief Pharmacist.

v Unit Dose Pharmacist

Ø Unit Dose
Pharmacist will print the Patient's Unit Dose Report as his/her reference on
charting prior to charging.

Ø Unit Dose
Report shall be updated against the patient's Medication Administration Guide as
he/she goes on rounds for patient chart review on each in-patient under charge
account.

Ø Records
all standing oral medication orders, their dose, dosage form, duration, and
timing of each drug.

Ø Takes notes and pulls out drugs from the patient's drug cubicle that were discontinued, held,
shifted, and revised then issues a credit memo form for the returned
medications.

Ø Counts the
remaining drugs and charges medications good for 24 hours.

Ø Pack
medications per patient per room and delivers to each nursing care unit, except
special areas.

Ø Perform
other necessary duties that may be assigned by the Senior Pharmacist or Chief
Pharmacist.

III.
EDUCATIONAL
BACKGROUND QUALIFICATIONS:

Ø Graduate of Bachelor of Science in Pharmacy.

Ø With a valid and updated PRC license

Ø Holds FDA Drug Licensing Seminar Certificate

IV.
EXPERIENCE
NEEDED TO PERFORM THE POSITION:

Ø Having at least one (1) year of work experience is
an advantage but not preferred

Ø Computer literate

Ø Physically, mentally, and morally fit to work
