Clinical Pharmacist

Responsibilities

• Review and evaluate physician orders to ensure appropriateness, safety, and effectiveness of prescribed medications.
• Provide clinical recommendations on drug therapy, dosage adjustments, and alternative medications based on patient conditions.
• Monitor patient responses to drug therapy, detect adverse drug reactions, and recommend interventions.
• Collaborate with physicians, nurses, and other healthcare providers in multidisciplinary rounds to optimize pharmacotherapy.
• Provide drug information, counseling, and education to patients and their families regarding medication use, side effects, and adherence.
• Ensure safe preparation, compounding, and dispensing of medications according to established standards and protocols.
• Conduct medication reconciliation during patient admission, transfer, and discharge.
• Participate in antimicrobial stewardship programs, therapeutic drug monitoring, and pharmacovigilance activities.
• Assist in the development and implementation of hospital policies, clinical guidelines, and formulary management.
• Train and mentor pharmacy interns, staff pharmacists, and other healthcare professionals in clinical pharmacology.
• Stay updated with current trends, evidence-based practices, and regulatory requirements in clinical pharmacy.

Qualifications

• Bachelor of Science in Pharmacy (BSPharm) or Doctor of Pharmacy (PharmD) degree.
• Must be a Licensed Pharmacist with a valid PRC license.
• Preferably with hospital pharmacy or clinical pharmacy experience (1–2 years minimum).
• Strong knowledge of pharmacokinetics, pharmacodynamics, and therapeutic drug monitoring.
• Excellent clinical decision-making, problem-solving, and communication skills.
• Ability to work collaboratively in a multidisciplinary healthcare team.
• Proficient in medication safety standards, hospital formulary systems, and electronic health records.
• Willingness to work in a hospital setting with shifting schedules.

Pay: Php20, Php25,000.00 per month
