Responsibilities
- Review and evaluate physician orders to ensure appropriateness, safety, and effectiveness of prescribed medications.
- Provide clinical recommendations on drug therapy, dosage adjustments, and alternative medications based on patient conditions.
- Monitor patient responses to drug therapy, detect adverse drug reactions, and recommend interventions.
- Collaborate with physicians, nurses, and other healthcare providers in multidisciplinary rounds to optimize pharmacotherapy.
- Provide drug information, counseling, and education to patients and their families regarding medication use, side effects, and adherence.
- Ensure safe preparation, compounding, and dispensing of medications according to established standards and protocols.
- Conduct medication reconciliation during patient admission, transfer, and discharge.
- Participate in antimicrobial stewardship programs, therapeutic drug monitoring, and pharmacovigilance activities.
- Assist in the development and implementation of hospital policies, clinical guidelines, and formulary management.
- Train and mentor pharmacy interns, staff pharmacists, and other healthcare professionals in clinical pharmacology.
- Stay updated with current trends, evidence-based practices, and regulatory requirements in clinical pharmacy.
Qualifications
- Bachelor of Science in Pharmacy (BSPharm) or Doctor of Pharmacy (PharmD) degree.
- Must be a Licensed Pharmacist with a valid PRC license.
- Preferably with hospital pharmacy or clinical pharmacy experience (1–2 years minimum).
- Strong knowledge of pharmacokinetics, pharmacodynamics, and therapeutic drug monitoring.
- Excellent clinical decision-making, problem-solving, and communication skills.
- Ability to work collaboratively in a multidisciplinary healthcare team.
- Proficient in medication safety standards, hospital formulary systems, and electronic health records.
- Willingness to work in a hospital setting with shifting schedules.
Job Types: Full-time, Permanent
Pay: Php20, Php25,000.00 per month
