Ward Clerk

1. Acts as receptionist in the nursing unit.

A. Greet patient's relatives, visitors and doctors with a smile as they come and give due courtesy and attention.

B. Give accurate directions and non-medical information to visitors.

C. Answers calls promptly and courteously.

1. Performs clerical and other related unit's activities.

A. Copies data/doctor's order from patient's chart to Kardex and submit all transcribed doctors' order to the Nurse Unit Manager for checking.

B. Encodes requisition for lab test, and diagnostic procedures and let nurse in charge counterchecked all encoded requisition.

C. Fill out the headings on each patient's medical record by checking if ID sticker label bearing patient's name PIN and name of attending physician, was placed on every forms in patient's medical record.

D. Receives telephone call but will refrain from receiving neither doctor's order nor doctor's call asking for patient's medical condition. Channel calls receive properly. Pass the phone tactfully to inform the nurse unit manager or charge nurse of the doctor's call.

E. Give pertinent information correctly, promptly and with utmost courtesy.

F. May encode and transcribed diet instruction in the HIS and Kardex.

G. Follow-up results of all diagnostic procedures and laboratory works of patient and incorporate in the correct chart if instructed.

H. Sends out fax request / PAC for diagnostic procedures and test to the other departments. Checks if the department received the request.

I. Verify stock supplies of unit and prepare requisition to maintain established inventories

J. Keep file of medical records of in-patient and out-patients in a locked drawer.

K. May record and compute census of patient in the census logbook.

L. Check whereabouts of chart when patient is transferred or dismissed following specified procedure outside of their unit or of the hospital to ensure that the patient's medical record/chart is returned to the unit concerned.

M. Convey message to other department as requested by the Nurse Unit Manager or Charge Nurse for the shift.
