Job description:

1.0 Duties and Responsibilities:

A. Patient Care

  1. Assists in the admission of patients into the unit.

  2. Makes general nursing assessment of patients conditions:

· Takes vital signs and records properly

· Notes physical appearance and capacity of patients

· Identifies nursing problems

· Institutes appropriate plan of care

· Evaluates action

· Records reports observation and care rendered

  1. Initiates hemodialysis of patients

· Cannulates the AV Fistula

· Opens and connects AV shunt to bloodlines

  1. Carries out doctor's prescriptions/therapeutic managements

  2. Computes for and monitors ultra-filtration rate

  3. Observes for signs and symptoms of complications and administers appropriate management and care

  4. Refers to Supervising/Attending Nephrologist for problems

  5. Terminates the dialysis procedure

  6. Explains the procedures and treatments given to patients

  7. Ensures the safety of patients during the procedure.

B. Unit Management

  1. Assists the Dialysis Nurse in maintaining adequate supplies, instruments, equipment and drugs of the unit

  2. Assists the Dialysis Nurse in maintaining a clean, comfortable, safe working area.

C. Staff Development

  1. Attends seminars, in-service training, lectures, initiated by the health care facility

  2. Attends unit meetings.

D. Role in Patient and Family Rights Responsibility

  1. Actively support Patient's Rights and Responsibilities policy and procedure of the organization.

  2. Ensure that all patients are aware and educated about their rights and responsibilities in accordance to the hospital policy.

  3. Treats patients and their families with respect and dignity. Identifies and addresses psychosocial, cultural, ethnic and religious/spiritual needs of patients and their families. Functions as liaison between administration, patients, physicians and other healthcare providers.

  4. Interacts professionally with patient/family and involves patient/family in the formation of the plan of care.

Educational Qualification and Licensures:

1.1 Diploma or Bachelor Degree in Nursing in an accredited college or university.

1.2 Valid PRC ID

1.3 Current Basic Life Support (BLS) Certificate, with Advance Cardiac Life Support (ACLS) and IVT preferred.
