Job Responsibilities

• Audit daily notes for proper documentation and ensure chart reviews are timely, accurate, and compliant with all CMS data requirements.
• Collaborate with field clinicians providing in-home patient care.
• Review charts to assess the quality of care delivered, ensuring orders are followed and appropriate communication occurs with physicians and other disciplines.
• Monitor compliance with internal standards and regulatory requirements, making recommendations for adjustments to company programs as needed. Develop and implement action plans to correct identified deficiencies.
• Stay up-to-date on clinical programs, processes, and changes.
• Work directly with the DON to coordinate quality improvement activities, including analyzing, reviewing, and reporting on notes, charts, charting trends, and quality assurance indicators. (Kinnser/WellSky is the primary patient data management system used.)
• Identify peer-review cases through internal and external sources as appropriate. Participate in planning, implementing, and evaluating departmental activities. Assist the DON and staff in evaluating programs and implement changes to ensure program improvement and regulatory compliance.
• Communicate updates and collaborate with the supervising nurse and external clinical staff regarding necessary changes in patient treatment plans.
• Monitor compliance with policies and procedures, including (but not limited to) clinical documentation standards.
• Resolve charting issues and recommend improvements to procedures.
• Serve as a consultant in implementing chart review tools, standards of care, and outcome measures.
• Attend case conferences and present audit findings to staff.
• Perform other related duties as assigned.

Qualifications

• Minimum of 2 years of clinical or related experience.
• Registered Nurse

• Excellent oral and written communication skills.

• Experience in clinical documentation review.

• 9:00 AM – 6:00 PM Pacific Time (PT)
