Key Responsibilities
  • Provide direct nursing care including vital sign monitoring, medication administration, wound care, and other prescribed treatments.
  • Monitor and record changes in the client's health, with special attention to CKD-related conditions, reporting concerns to physicians or family members as appropriate.
  • Assist with daily living activities such as mobility, personal hygiene, and nutrition support, ensuring renal-friendly dietary management.
  • Coordinate and attend medical appointments, including nephrology visits and dialysis sessions, ensuring continuity of care and accurate communication with healthcare providers.
  • Manage medications: organizing prescriptions, tracking dosages, and preventing errors, particularly for CKD-related treatment plans.
  • Develop and implement personalized care plans based on physician orders and the unique needs of a CKD patient.
  • Maintain infection control, safety, and hygiene standards within the client's living environment.
  • Offer emotional support and companionship to promote overall well-being.
  • Provide health education and guidance to the client and their family members regarding CKD management and lifestyle modifications.

Qualifications & Skills
  • Registered Nurse (RN) or Licensed Practical Nurse (LPN) with valid state license.
  • Proven clinical experience in private duty nursing, home healthcare, or hospital settings; prior experience with CKD or dialysis patients strongly preferred.
  • Strong knowledge of medical terminology, nursing procedures, and patient care techniques, particularly renal care.
  • Excellent communication and interpersonal skills.
  • Ability to work independently, exercise sound judgment, and handle emergency situations calmly.
  • Compassion, patience, and discretion when working in private households.
  • Basic life support (BLS) and/or advanced cardiac life support (ACLS) certification preferred.
