Job Summary

The Pharmacist is responsible for patient care involving outpatient parenteral antimicrobial therapy (OPAT), as well as managing the procurement of necessary supplies and medications to ensure the effective operation of the clinic. This role includes collaboration with healthcare professionals and suppliers, patient education, and ongoing quality improvement efforts to enhance patient outcomes.  The role plays a critical part in ensuring the success of the OPATHealth by balancing clinical responsibilities and operational efficiency.

Job Responsibilities

  • Evaluate medication orders for appropriateness and accuracy.
  • Provide evidence-based recommendations for antimicrobial therapy.
  • Monitor patient progress and therapeutic outcomes.
  • Counsel patients and their families regarding medication use, administration, and potential side effects.
  • Develop individualized care plans in collaboration with the healthcare team.
  • Conduct follow-up assessments to ensure the effectiveness of therapy.
  • Collaborate with physicians, nurses, and other healthcare providers in developing treatment plans.
  • Participate in multidisciplinary rounds to discuss patient cases.
  • Share relevant clinical information with team members to enhance patient outcomes.
  • Collaborates with suppliers and pharmacy teams to optimize procurement processes and manage stock levels efficiency
  • Participate in quality assurance and improvement initiatives within the OPAT program.
  • Monitor for and report adverse drug reactions or medication errors.
  • Implement and adhere to safety protocols and best practices in medication management.
  • Effectively communicate medication-related information to patients and healthcare team members.
  • Document clinical interventions and patient interactions accurately and comprehensively.
  • Provide education to clinical staff on antimicrobial stewardship and best practices.
  • Manage clinical challenges related to patient care and medication management.
  • Adjust treatment plans based on patient response and emerging evidence.
  • Address issues that arise in the clinic, demonstrating flexibility in approach.
  • Ensures timely procurement of medications and supplies to maintain clinic operations, including inventory management and oversight
  • Manage clinic inventory by tracking medication usage and ensuring that the necessary supplies are procured and readily available
