Patient Intake Coordinator

Job Overview: Receives and processes patient admission into our agencys EMR system for homehealth services.Coordinate care by assigning case manager and clinician to ensure 48 hour admissionFollow up on inquiries related to the patient admission and staff availability.Assists in establishing the initial Plan of Care and plotting of tasks to staff schedule.Perform coordination duties to support day to day work flow of our field staff Responsibilities: Providing outstanding customer serviceAnswering inbound calls and explaining servicesScheduling/rescheduling appointments and managing 15+ calendarsSupporting the Placement Team with staffing requestsAssisting nursing, therapists and administrative team as needed Qualifications: Must have completed at least 2 years of college educationGood English communication skills - both written and verbalStrong phone skills for inbound/outbound callingHospital or medical office experience preferredFamiliarity with medical terminology an advantageAbility to work with computers and different software packages (e.g. MS Office, AdobeAcrobat)Ready to take new responsibilities and grow professionallyWilling to ask questions and work as a part of the team, even remotely locatedAvailable for occasional overtime workWilling to start ASAPWilling to work full-time night shiftWilling to work on weekendsLooking for a long-term position Work Schedule: Night Shift Select VoiceCom is an American and Australian-owned company providing call center services.
