Job Title: Patient Care Coordinator

We are seeking a compassionate and dedicated patient advocate to help patients navigate the complexities of the healthcare system. As a patient care coordinator, you will play a vital role in connecting individuals with financial assistance programs and resources they need to access essential medical care.

Our mission is to make healthcare more accessible for everyone by providing compassionate support and efficient processes. We work closely with healthcare providers, pharmacies, and financial aid programs to streamline assistance and improve patient outcomes.

• Enroll patients in philanthropic aid programs.
• Effectively manage post-enrollment work such as submitting claims or ordering medications.
• Review patient enrollments for accuracy and completeness.
• Escalate and appeal patient assistance denials.
• Order, receive, and log medication orders.
• Coordinate medication deliveries to the designated pharmacy timely.
• Provide exceptional customer service to both the patient and client.
• Offer emotional support and empathy to patients.

• Proven experience in a high-performance environment.
• Hands-on experience working in healthcare is required.
• High school diploma or equivalent.
• Strong listening and communication skills.
• Excellent written and verbal communication skills in English.
• Proficient in computer skills.
• Attention to detail.
• Ability to offer emotional support and empathy.
• Have a passion for the well-being of patients.
• Pass a standard background check and drug screen.
• Complete medical clearance per health system requirements.
• Remote positions require a distraction-free office environment that is HIPAA-compliant.

• Internet access provided by a cable or fiber provider.
• Minimum upload speed of 2 Mbps.
• Minimum download speed of 5 Mbps.
• Maintaining an uninterrupted internet connection is a requirement of the Work From Home position.

A modem/router combination can cause connection problems with company-issued phones. You may need to add a router to your network that meets the following guidelines:

• Ethernet ports wired for gigabit speed connections.
• Availability of at least two Ethernet connections to the internet source is required.
• Working wirelessly is prohibited.
