Intake Coordinator - Home Health Care Job Summary

We are seeking a skilled professional to manage the intake process for new patients in home healthcare operations.

  1. Coordinate the intake process ensuring compliance with regulations and high-quality patient care.
  2. Review, write and edit clinical notes in collaboration with physicians and nurses.
  3. Track referrals, authorization statuses, and admission workflows daily to ensure timely patient care.
  4. Support all administrative operations including scheduling, documentation, and internal communications.
  5. Serve as liaison between field clinicians and doctors to streamline documentation and provide seamless patient care.

Requirements:

  • A minimum of 3-5 years experience in Intake Coordination in the United States preferably within California's home health sector.
  • Clinical credentials include Registered Nurse (RN), Doctor (MD), or equivalent healthcare expertise.
  • Excellent English communication skills written and spoken with a clear and understandable accent.
  • Proven hands-on experience with DataSoft Logic and Axxess platforms.
