Job Opportunity: Senior Case Manager

We are seeking a highly skilled and experienced Senior Case Manager to join our team. As a Senior Case Manager, you will play a critical role in delivering evidence-informed case management and individualized support to individuals in the community.

The successful candidate will have a strong understanding of case management principles and practices, with a focus on safety, risk assessment, and rehabilitation. You will be responsible for providing case management services to adults subject to community orders, including conducting risk assessments, developing and implementing case plans, and monitoring progress.

Key Responsibilities:

  • Provide case management services to adults subject to community orders, focusing on safety, risk assessment, and rehabilitation.
  • Conduct risk assessments and develop and implement case plans that meet the needs of individuals under community-based orders.
  • Monitor progress and provide ongoing support to individuals under supervision.
  • Consult with relevant Aboriginal and Torres Strait Islander communities and organisations to support culturally informed practices.
Requirements
  • Possession of a driver's licence (minimum C class Australian licence) is mandatory as travel across various locations is required.
  • May be required to work hours outside the normal work hours and travel to other locations, or temporarily work from an alternate location.
  • Individuals employed on a permanent or temporary basis will be required to complete Certificate IV in Correctional Practice (Community).
