Job SummarySocial Work Assistants assist healthcare teams in planning and implementing programs to meet the social and emotional needs of patients and patients' families. They develop plans for patient care after release from enrollment until postpartum care period.
Key Responsibilities:Setup and maintain all aspects of the Social Work Program including case reports, program reports, and budget allocation.
Establish strong case reports through home visits, hospital visits, and other patient-assistive activities outside of our organization.
Develop education modules for selected patient curricula.
Facilitate orientation for new patients during initial interviews.
Establish partnerships with non-profit organizations, government entities, and private institutions to increase services provided.
Coordinate patient visits for patients transported to the hospital.
Participate in monthly meetings, outreaches, and clinical activities with a positive attitude.
Submit monthly accomplishment reports to the SW Manager.
Document patient stories (1-2 stories) and ensure patient charts are complete on SW forms.
Assist patients in emergency situations, process paperwork, and perform other activities as needed.
Maintain regular updates on DSWD registration and accreditation of our organization.
Sign off medical charts of discharged patients to ensure completeness.
Provide TeleHealth care via phone, Zoom, and social media platforms in accordance with government regulations.
Perform other functions assigned by superiors or the CEO.
Respond to emails sent by Management or Department and keep up-to-date on departmental policies, changes, and events.
Licensed Social Worker required.
Maintain confidentiality and communicate clearly and concisely in written and oral form.
Perform clerical and social work tasks quickly and accurately.
Coordinate daily activities and schedule.
Work independently and organize complex tasks.
Remain calm under pressure and adapt to various situations.
Maintain efficient office procedures and track requested actions and reports.
Meet strict timelines and perform multiple tasks.
Work harmoniously with colleagues and patients.
Operate office equipment efficiently.
Proficient in technology use, including computers, word processing, database, spreadsheet programs, and PowerPoint.
Promote proactive communication among team members and patients.
Accomplish responsibilities with minimal supervision.
Knowledge of departmental functions and biblical principles is required.
