
Medical Documentation Specialist

Job Overview:
The Medical Scribe VA is responsible for providing accurate and timely documentation of patient encounters. This role involves real-time scribing, capturing relevant history, objective findings, assessments, treatment plans, and outcomes in clear, structured notes.
Key Responsibilities:
- Accurately document patient encounters directly into the Electronic Health Record (EHR) during or immediately after visits.
- Capture relevant information in a clear and concise manner, using clinic templates and standards to ensure notes are billable, compliant, and ready for provider sign-off.
- Maintain consistency of records across encounters and ensure timely chart updates.
Requirements:
- Proven experience as a medical scribe, medical virtual assistant, or healthcare documentation specialist.
- Strong written and spoken communication skills, with the ability to communicate effectively with patients, providers, and other healthcare team members.
- Typing speed of at least 45 WPM with high accuracy.
- Working knowledge of basic medical terminology.
- Proficiency with EHR documentation and Microsoft Office, with strict adherence to HIPAA guidelines.
