We are seeking a highly skilled Care Navigator to join our team. The ideal candidate will have exceptional patient care coordination skills, experience in healthcare sales, and strong communication abilities.

As a Care Navigator, you will play a critical role in ensuring seamless patient experiences. Your primary responsibilities will include:

Key Responsibilities:
  1. Ensure professional patient interactions as the first point of contact.
  2. Collect, verify, and update patient information while maintaining confidentiality.
  3. Schedule appointments efficiently and conduct initial screenings.
  4. Manage patient records in EMR systems, adhering to HIPAA regulations.
  5. Respond to patient inquiries via phone, email, and chat with empathy and clarity.
  6. Provide pre-appointment instructions and ensure patients understand documentation and insurance requirements.
  7. Coordinate with providers, insurance teams, and staff to ensure smooth patient flow.

In addition to these responsibilities, you will also be expected to:

Additional Duties:
  1. Conduct provider outreach and present our value proposition to acquire provider partnerships.
  2. Maintain accurate outreach records in CRM systems.
  3. Track and report key performance indicators (KPIs).
