Job Description

Our organization provides case management services to support the safety, stability, and well-being of children and families during the referral period. We are seeking a skilled professional to deliver culturally competent, strengths-based case management services consistent with evidence-based practice models.

Responsibilities:
  • Meet deadlines for connecting with families within 24 hours of referral; facilitate initial family meetings within 48 hours; complete assessments-based case plans within 20 days; and close cases within 3-6 months.
  • Engage and work in partnership with families to complete assessments and develop case plan goals and objectives.
  • Meet with families in their homes weekly to monitor progress toward case plan goals and meet monthly with each child individually to assess child safety.
  • Assess child safety with every family contact and report suspected child abuse/neglect.
  • Monitor family progress and service provision using assessment tools and update the case plan accordingly.
  • Provide home-based family skills training to assist parents in providing a safe environment for their children.
  • Work closely with the Family Support Worker to provide services and make decisions about child well-being.
  • Prioritize problems requiring resolution, establish goals and objectives, utilize available resources, and make informed decisions.
Required Skills and Qualifications:
  • Bachelor's degree in human services is required.
  • Experience working in behavioral health or children and family services is necessary.
  • Substance use experience is preferred.
  • Valid driver's license, automobile liability insurance, and a Motor Vehicle Record meeting organizational standards are required.
