An Advanced Care Nurse is a highly skilled and dedicated healthcare professional who works in various medical settings to provide direct patient care, including assessment, planning, implementation, and evaluation of patient care plans. In this role, the individual will be responsible for delivering high-quality nursing services to patients with diverse needs, working collaboratively with healthcare teams to ensure seamless and effective patient care.

Responsibilities and Duties
  • Patient Assessment and Care Planning: Conduct thorough assessments of patients' physical and emotional needs, developing and implementing personalized care plans that address their unique requirements.
  • Nursing Care Delivery: Provide direct patient care, administering medications, treatments, and performing various medical procedures as required.
  • Collaboration and Communication: Work closely with healthcare teams, including physicians, therapists, and other support staff, to ensure coordinated and effective patient care.
  • Education and Support: Educate patients and their families about health conditions, treatment options, and self-care techniques to promote independence and well-being.
  • Documentation and Record-Keeping: Maintain accurate and comprehensive patient records, including nursing assessments, interventions, and outcomes.
Qualifications and Requirements
  • Bachelor's degree in Nursing (B.S.N.) or equivalent experience.
  • Current Basic Life Support (BLS) certification and Advanced Cardiovascular Life Support (ACLS) training preferred.
  • Ability to travel and work in different locations within designated areas.
  • Excellent communication, interpersonal, and problem-solving skills are essential for success in this role.
