Job Title: Patient Mobilization Coordinator
This role is responsible for connecting patients, their families, and healthcare providers to enhance the patient experience while at our facility. Key responsibilities include organizing activities outside of the hospital to assist in building networks and partnerships, creating focal contact persons in local government units and private institutions, and assisting patients during clinic appointments.
Essential Duties:
- Establish relationships with local organizations, educational institutions, and community-based groups to increase awareness of our programs and services.
- Coordinate with the Fieldwork Head to ensure deliverables, goals, and projects are met.
- Assist with planning and execution of mobile clinics and other community-based events.
- Support patients in finding and completing necessary paperwork, arranging transportation, and addressing other treatment-related issues.
- Follow up with patients to enhance recovery pathways and address past surgery concerns.
- Communicate effectively with staff members to ensure proper services are provided.
- Prepare regular activity reports summarizing accomplishments.
- Perform other duties as assigned by the supervisor to further the organization's mission.
Other Responsibilities:
- Adhere to high standards of quality and efficiency in line with hospital requirements.
- Ensure a safe work environment by following infection prevention protocols.
- Commit to sustainable work practices by minimizing waste and optimizing resource usage.
Education/Experience Requirements:
- Minimum Education Required: Bachelor's degree in Social Work or a related field.
- Certificate/License Required: Professional Driver's License.
- Minimum Experience Required: 1-2 years of experience in social work, nursing, or community development, preferably in a healthcare organization or non-profit setting.

- Strong negotiation and organizational skills.
- Ability to build rapport with people and create a positive working relationship.
