Substance Use Disorder Program Coordinator | Night Shift | Ortigas

This position is responsible for coordinating with the Program Manager to ensure client engagement and measurement data is tracked in accordance with the needs of the program and its anticipated goals. The Coordinator supports the Substance Use Disorders (SUDS) program by ensuring tasks are completed, and clients have been contacted and educated about the available services. They provide leadership to the team of SUDS Patient Support Specialists as the department expands to meet the needs in the communities we serve.
Position Responsibilities include, but are not limited to the following:
1. Perform outreach to the provided target population list to educate clients on services offered for substance use disorder services and programs daily.
2. Link clients to appropriate SUDS services while monitoring Provider schedules and ensure they are up-to-date and maximized. Additionally, engaging clients in services needed such as IOP, Therapy, and Psychiatry, as appropriate.
a. Educate the client on the different services and programs for SUDS clients at WC Health and determine the linkage depending on appropriateness.
b. Ensure that the team is scheduling patients with Providers according to accepted/credentialed insurances and specific Provider preferences.
c. Manage the schedule effectively to link clients to the soonest appointments as are available.
d. Attempt to fill the Providers' schedules when absences occur, in real time, as much as possible.
e. Troubleshoot any medication and appointment scheduling issues with SUDS clients, as they arise.
3. Ongoing management of calls where the caller is identified as 'in crisis', as soon as possible, to ensure safe outcomes whenever possible.
4. Coordination of Care for non-intervention population of SUDS participants
a. Monitor emails and respond within 24 hrs, even if only to acknowledge receipt.
b. Ensure all referrals are documented and communicated with the SUDS team for follow-up.
c. Ensure missed appointments are called daily for rescheduling by team members.
d. Ensure that calls to confirm scheduled appointments are completed the day before.
e. Ensure all required program documents are present, accurate, and correctly added to the documents section of the patient chart.
f. Review voicemails, delegating, and returning missed phone calls on the same day, when possible for all SUDS clients.
5. Collect and enter data relating to Inputting all measurements and data points needed for internal and external reporting.
a. Gather data for all non-intervention SUDS clients for input to a variety of platforms as they enter the program and ongoing until discharge.
b. Data assistance for SUDS intervention clients on multiple platforms as needed.
Knowledge, Skills, and Abilities preferred:
. Experience working with behavioral health programs, counseling, and/or mental health services as well as those treating Substance Use Disorders, for a minimum of two (2) years, (recommended).
. Strong organizational and leadership skills ability to multitask, to set and follow priorities, and ensure focus on attention to detail.
. Ability to thrive in a fast-paced environment with flexibility to manage changes, as they occur.
. A minimum of 3+ years experience in a health-related field, (mental or medical) required.
. Knowledge and skills in managing computer-based work including the use of Excel, Word, and Google products, or the ability to learn new/cutting-edge platforms or databases.
. Excellent communication skills, ability to delegate, and manage deadlines, as indicated.
. Use of Electronic Health Record (EHR) and online databases, as well as eligibility databases, and records management systems.
. Energetic, upbeat, and positive attitude with an interest in working in an environment that expects dedication and hard work.
Education and Experience:
. With nursing background
. Psychology background is a plus but not required
. Nursing graduate.
. Previous experience in healthcare administration or direct client care (preferred).
