Job Description:

  • Provides clinical support to ensure the safe, effective, and rational use of medications
  • Reviews and verifies physician orders for accuracy, appropriateness, and potential drug interactions
  • Participates in patient care rounds and collaborates with physicians and nurses to optimize pharmacotherapy
  • Monitors patient responses to medications and recommends necessary adjustments
  • Provides drug information, counseling, and education to healthcare professionals and patients
  • Supports the hospital's Antimicrobial Stewardship and Medication Safety Programs
  • Prepares and dispenses medications in accordance with hospital policies and DOH/FDA standards
  • Conducts medication use evaluations and contributes to policy development
  • Ensures proper storage, documentation, and control of pharmaceutical products

Qualifications:

  • Bachelor of Science in Pharmacy or Doctor of Pharmacy (PharmD) graduate
  • Licensed Registered Pharmacist (RPh) with valid PRC license
  • Preferably with clinical or hospital pharmacy experience
  • Strong knowledge of pharmacology, drug interactions, and therapeutic guidelines
  • Excellent analytical, communication, and documentation skills
  • Able to work effectively in a multidisciplinary healthcare team
  • Detail-oriented, organized, and committed to patient safety and quality care
  • Willing to work on shifting schedules and continuous professional development

Job Types: Full-time, Permanent

Pay: Php20, Php25,000.00 per month
