SPECIFIC DUTIES AND RESPONSIBILITIES

a. Works under the direction of a licensed nurse (Under no circumstances shall they work independently without the supervision of, or monitoring by, a licensed nurse who provides assessment of clients )

b. Listens to shift report, obtains assignments and the condition of the patient from the Head Nurse

c. Reports to the Head Nurse when leaving and returning the unit

d. Timely reports findings/observations to charge nurse RN responsible to the patient.

e. Determines absence of pulse and/or respiration and initiate an emergency response

f. Provides direct patient care and assist clients with the following tasks related to activities of daily living:

Tasks associated with personal care:

1) Bathing; dressing; grooming; shaving; shampooing and caring for hair

2) Providing and assisting with oral hygiene and denture care

3) Caring for the skin

4) Caring for the nails

5) Providing peri care

6) Bed making and handling linen

7) Maintaining environmental cleanliness

Tasks associated with maintaining mobility:

1) Ambulating, transferring, transporting, positioning, turning and lifting;

2) Elevating extremities;

3) Performing range of motion exercises

4) Maintaining alignment.

Tasks associated with nutrition and hydration:

1) Feeding and assisting client with eating and drinking.

Tasks associated with elimination:

1) Toileting assisting with use of bed pan and urinal;

2) Providing catheter care, including the application of and removal of external urinary catheters (condom catheter);

3) Administering enemas

4) Collecting specimens;

5) Emptying ostomy bags or changing ostomy bags that do not adhere to the skin;

Tasks associated with use of assistive devices:

1) Caring for dentures, eyeglasses and hearing aids;

2) Caring for, applying and removing anti embolus stockings, prosthetic devices, orthotic devices and braces;

3) Assisting with wheelchairs, walkers, or crutches; and using footboards;

4) Assisting with and encouraging the use of self-help devices for eating grooming and other personal care tasks;

5) Utilizing and assisting clients with devices for transferring, ambulation and alignment.

Tasks associated with maintaining environment and client safety.

1) Securing patient belongings.

2) Applying restraints/medical assistive devices (i.e., wrist restraints, vests, side rails.

3) Following hospital infection control policies.

4) Answering calls according to response time standards.

5) Ensuring identification band/bracelet is on each patient.

6) Maintaining clutter-free environment.

Tasks associated with data gathering, recording and reporting:

1) Measuring temperature, pulse, respiration and blood pressure;

2) Measuring height and weight;

3) Measuring and recording oral intake;

4) Measuring and recording urinary output, both voided and from urinary drainage systems;

5) Measuring and recording emesis;

6) Measuring and recording liquid stool;

7) Measuring and recording pulse oximetry; and

8) Collecting responses to pain using a facility- approved pain scale.

g. Performs indirect patient care

1) Assists in admission

a) Ensures room availability and readiness

b) Orients patient and family to room, unit, manipulation of equipment, hospital amenities.

c) Accommodates patient request without compromising hospital policies and procedures

2) Assists on transfer

a) Coordinates with bedside nurse if receiving unit is ready to receive patient

b) Informs patient and family time of transfer

c) Assists patient family in transferring their belongings/articles to chosen room.

d) Ensures that unit property or equipment per room is complete and in good condition just before patient vacate the room.

3) Assists in discharge

a) Coordinates with bedside nurse if discharge processing was initiated.

b) Assembles medical record and admin requirement at time of discharge and delivers them to registrar.

c) Assist patient and family in proper discharge clearance

d) Ensures proper filling up and collection of patient satisfaction survey

e) Ensures that unit property or equipment per room is complete and in good condition just before patient vacate the room.

Tasks associated with other related duties as assigned

  1. Promptly locates needed supplies and materials equipment needed by physician/nurse/health care team/patient

  2. Maintains unit housekeeping at all times

  3. Acts as runner or transporter of the health care team

  4. Reports head nurse and to concern department malfunctioning equipment

  5. Sets up oxygen administration set and suction apparatus

  6. Sets up Balkan frame efficiently.

  7. Performs inventory checks and generates requests and replenishment of the unit medical supplies/stocks to include admin supplies and forms to ensure functional level of supplies

  8. Monitors/records/charges equipment and supplies used by patient

  9. Ensures proper accounting of supplies and equipment.

  10. Facilitates detailed accurate endorsement to receiving nursing attendant and /or ward man.

  11. Delivers specimen and picks up medication (stat) and other items.

  12. Participates in the training and development of new staff and trainees.

  13. Communicates with patient's family, physicians and unit personnel and peers in an appropriate and courteous and professional manner.

  14. Assumes clerical assistant responsibilities when needed.
