About the Job:The RN Navigator – Telehealth is responsible for coordinating and supporting patient care before, during, and after telehealth visits. This position ensures patients are well-prepared for virtual consultations, assists with technical and clinical readiness, and supports providers throughout the visit to ensure efficient, high-quality care delivery.
Key Responsibilities:Pre-Visit Preparation:
- Arrive prior to telehealth sessions to prepare for the day's schedule.
- Contact and prepare patients for their telehealth visits, ensuring readiness and comfort with technology.
- Assist patients with logging in and troubleshooting connection or platform issues.
- Verify and update patient demographics, insurance information, and primary/specialty care team details in the EMR.
- Review and reconcile current medications; note any refill needs for provider review.
- Confirm completion of required pre-visit questionnaires, forms, and consents.
- Ensure all pre-visit documentation and paperwork are completed and uploaded.
During Visit:
- Notify provider when patient is ready and present a concise pre-visit read-out, including pertinent updates or concerns.
- Support provider during the telehealth visit, including entering orders, documenting provider instructions, and managing real-time updates in the EMR.
Facilitate referrals and coordinate follow-up appointments as directed by the provider.
- Initiate prior authorizations for medications, procedures, or referrals as needed.
Post-Visit Coordination:
- Review and close encounter documentation, ensuring completeness and accuracy.
- Communicate follow-up instructions and ensure continuity of care across the patient's care team.
- Monitor and track pending authorizations, test results, and referral completion.
- Provide ongoing patient education and navigation support to promote engagement and adherence to care plans.
- Complete Medication Refill Requests
- Education: Graduate of an accredited School of Nursing
- Licensure: Current, active PHRN license
- Experience: Minimum of 2 years of clinical nursing experience; with telehealth or care coordination experience
Skills:
- Strong communication, organizational, and critical thinking skills.
- Proficiency with EMR systems and telehealth platforms.
- Ability to educate and guide patients in a virtual environment.
- Knowledge of insurance verification, authorizations, and care coordination workflows.
