Educational Requirements

Duties and responsibilities:
Help patients develop coping strategies and resilience.
Coordinate discharge planning, ensuring patients have access to necessary resources and services after leaving the hospital.
Advocate for patients' rights and access to care.
Support patients in understanding their treatment options and making informed decisions.
Provide immediate support to patients and families in crisis situations, such as severe illness, death, or trauma.
Help de-escalate emotional distress and stabilize situations.
Connect patients and families with community resources, such as financial assistance programs, or counseling services.
Educate patients about government or community benefits they may qualify for.
Adhere to professional ethical standards, including confidentiality and informed consent.
Maintain accurate records of patient interactions, assessments, and interventions.
Prepare reports for healthcare teams or external agencies as needed.
