Educational Requirements

Licensure or Registration

  • Must be a Registered Social Worker

Duties and responsibilities:

  • Help patients develop coping strategies and resilience.

  • Coordinate discharge planning, ensuring patients have access to necessary resources and services after leaving the hospital.

  • Advocate for patients' rights and access to care.

  • Support patients in understanding their treatment options and making informed decisions.

  • Provide immediate support to patients and families in crisis situations, such as severe illness, death, or trauma.

  • Help de-escalate emotional distress and stabilize situations.

  • Connect patients and families with community resources, such as financial assistance programs, or counseling services.

  • Educate patients about government or community benefits they may qualify for.

  • Adhere to professional ethical standards, including confidentiality and informed consent.

  • Maintain accurate records of patient interactions, assessments, and interventions.

  • Prepare reports for healthcare teams or external agencies as needed.
