JOB TITLE: Hemodialysis Senior Staff Nurse

JOB QUALIFICATIONS:

· Must be a registered nurse

· With formal training as a hemodialysis nurse

· Must have one (2) year clinical experience as Renal Nurse

· Must be a Renal Nurse Association of the Philippines (RENAP) member

· Must be a Certified Nephrology Nurse by RENAP

· With updated IV therapy training/license, BLS and ACLS certification

JOB SUMMARY:

a. Assists patients with kidney failure through hemodialysis treatment.

b. Initiates, monitors, and terminates hemodialysis treatment.

c. Works closely with other medical staff to ensure that the patient is safe while undergoing hemodialysis treatment.

d. Refer any concerns, complications, laboratory values to the In-house Physician/Attending Nephrologist.

e. Discusses patient's concern and doing the health teachings to patient.

f. Verify that patients are taking all prescribed medications, advising doctors of any significant change in patient's health.

g. Reviews patient's laboratory schedules (Routine laboratories and hepatitis profile) and reminds them for next due schedule.

h. Supervision of patient care

i. Monitoring of clinical supplies

j. Ensures proper documentation

FUNCTIONS/DUTIES/RESPONSIBILITIES:

a. Pre-dialysis procedure

· Follows hemodialysis orders and treatment plan

· Verifies machine and dialyzer of the patient

· Weighs the patient

· Calculates amount of fluid to be removed

· Takes vital signs

· Assessment of patient's access

b. Initiation of dialysis

· Performs venipuncture of AVF/AVG access or proper catheter access care

· Sets hemodialysis parameters to hemodialysis machine

· Hooks patient to hemodialysis machine

c. Intradialytic Monitoring

· Monitors patient's status and his/her vital signs

· Records pertinent data to patient's chart

· Manages intradialytic complications and refers to in-house physician

d. Post-dialysis procedure

· Terminates procedure

· Takes vital signs

· Weighs the patient

· Complete and fill-out necessary data to patient's chart

E-Cart checking and monitoring

· Checks e-cart medicines and medical supplies weekly every Wednesday. These include medicines' and medical supplies' quantity and expiration date. Checks e-cart medical equipment (e.g. defibrillator, 12-L ECG machine, Ambu bag, etc.) integrity twice weekly (every Wednesday and Saturday) and date of next calibration.

· Informs Inventory Nurse and Nurse Manager within one (1) month of scheduled date of calibration of e-cart's medical equipment to notify the manufacturer/supplier that will conduct calibration.

· Reviews e-cart medications' and medical supplies' expiration date every first Wednesday of the month and gives the Nurse Manager for list of medications and medical supplies that will expire within six (6) months.

· Records medicines and medical supplies used from e-cart indicating date used, patient's name, medication/supply used, quantity, and name of nurse at E-Cart Logbook.

· Records replacement of medicines and medical supplies indicating medication's/supply's name, quantity, expiration date, name of nurse who received the medicine/supply and signature of Nurse Manager at E-cart Logbook.

Medical Supplies/Equipment inventory, maintenance, and storage

· Establish initial inventory of medical supplies, quantity and their corresponding expiration date, and medical equipment, quantity and their corresponding date of calibration (if applicable).

· Ensures proper labelling of medical supplies storage as well as proper placement and content of inventory card for each medical equipment

· Informs Nurse Manager within one (1) month (yearly) of scheduled date of calibration of medical equipment to notify manufacturer/supplier that will conduct calibration; collaborates with E-Cart Nurse for e-cart's medical equipment for calibration.

· Determines standard quantity of dispensed/"Out" medical supplies at nurse station for daily use; notifies other staff and Nurse Manager for any medical supplies misusage or over usage.

· Checks medical supplies inventory daily and accomplish Daily Inventory Form (For Nurses), determines next day's number of patients and requests additional medical supplies needed to the Nurse Manager.

· Collaborates with Re-use Technician's inventory of next day's needed supplies for patient's hemodialysis machine treatment use.

Supervision of patient care

· Performs the functions, duties, and responsibilities of a Hemodialysis Senior Staff Nurse.

· Ensures the faithful implementation of the center's policies, protocols, and procedures in providing quality patient care.

· Performs administrative work planning, coordinating, and directing nursing staff and hemodialysis technician in the center.

· Makes nursing rounds during start of every shift; checking patients' access, HD parameters, and extracorporeal blood circuit; designates staff their patient's assignment.

· Identifies nursing problems and assist in their solution.

· Uses hemodialysis treatment sheet as authoritative source to communicate significant information to all nursing staff

· Ensures accurate recording and reporting of patient's symptoms, reactions, observed progress and nursing action done by the nursing personnel to patient's chart.

· Ensures proper and complete chart documentation before returning it to the chart cabinet.

  • Directly reports and collaborates with Nurse Manager regarding care management of patients, matters regarding In-house physicians, and other medical concerns in the operations of the center.
  • Provides accountability and utilizes established standards and criteria for evaluating nursing care in the center.
  • Assists with the evaluation related for the improvement of the center's health care.
  • Collaborates with the Nurse Manager regarding supervision of the Junior Nursing staff and care management of patients.
  • Utilizes established standards and criteria for evaluating nursing care.

· Compiles the data for the quality assurance and sends a report to the nurse manager in preparation for the Quality Assurance Meeting with the Head Nephrologist.

· Secures patients' weekly schedule.

Senior Nurse 1. Medical records officer

· Collaborates with the Nurse Manager formation of forms, records, and documents necessary for DOH, Philhealth requirements and the center's operation.

· Establish an initial and maintains updated record of "Patients Master List".

· Ensures complete initial contents of patients' medical records at patient's chart (Refer to Medical Records Policy).

· Reviews completion of monitoring records and patient's chart weekly every Wednesday and notify related staff for any discrepancies.

· Facilitates de-bulking of patient's chart quarterly (every first Wednesday of January, April, July, and October); Ensures proper labelling, storage, and retention of records as per DOH guidelines (10 years).

· Conducts disposal of records as approved by Nurse Manager based on DOH guidelines and center's policy (Refer to Medical Records Policy).

· Maintains patient privacy by keeping patient records information confidential.

· Collates pertinent record such as the patients' schedule, monthly calendar of activities, daily inventory, Physicians' schedule, etc.

· Accomplishes "Daily Request" report and sends it to the nurse manager.

Senior Nurse 2. Compliance officer of the Philippine Renal Disease Registry (PRDR)/ Renal Disease Control Program (REDCOP) and Monthly Equipment Inventory

· Gathers important information for the registration of the REDCOP

· Ensures validity of the Certificate of Compliance

· Sends gathered information to the PRDR every quarter (January, April, July, October)

· Sends a report on the monthly medical equipment to the nurse manager.

Senior Nurse 3. Infection Control Officer

· Ensures that all staff, both medical and non-medical, knows and strictly follows the General Infection Control Guidelines.

· Responsibly empowers the Healthcare Team to orient the patients and patients' companion on the infection control guidelines of the unit.

· Ensures the proper use of PPE inside the health facility.

· Monitors the vaccination record of both medical and non-medical staff.

· Records and monitors the vaccination and hepatitis profile of the patients.

· Ensures completeness of both Influenza Vaccine (Annually) and Pneumococcal Vaccine (Every 5 years) for all staff members of the unit.

· Guarantees the overall cleanliness of the center. (Refer to Infectious Waste Protocol and Housekeeping Protocol).

· Educates the patients and staff members regarding the proper waste disposal.

· Sends a monthly report to the nurse manager regarding the hepatitis profile status of the patients.
