Nurse Care Coordinator

As a Care Coordinator, you will play a pivotal role in enhancing patient health outcomes within the United States. You will collaborate closely with physicians, specialists, and team members to develop tailored care plans, educate patients and caregivers on disease management, medications, and lifestyle adherence.
Key Responsibilities:- Serve as the primary care coordinator for assigned US-based patients
- Conduct telephonic outreach and/or in-person assessments (based on program guidelines)
- Collaborate with healthcare providers to create patient-centered care plans
- Educate patients and caregivers on chronic disease management
- Track key health indicators and intervene early to prevent ER visits or hospital readmissions
- Active RN license
- Experience in chronic care, patient education, or case management
- Excellent English communication skills
- Strong clinical judgment and attention to detail
