Remote Care Coordination Role

This is a dynamic remote opportunity for a skilled care coordinator to collaborate with our international team. The ideal candidate will possess excellent communication and documentation skills, with the ability to work independently and maintain accurate records.

Key Responsibilities:
  • Care Coordination:
    • Serve as the primary liaison between patients and caregivers, ensuring seamless care delivery and efficient patient flow.
    • Monitor and maintain care quality by completing quality assurance activities and reporting outcomes.
    • Communicate effectively with patients, families, physicians, and care teams regarding scheduling and plan-of-care updates.
  • Communication & Documentation:
    • Maintain accurate documentation for all caregivers and patients in electronic health records.
    • Log and maintain proper documentation of events and care-related activities in accordance with departmental procedures.
  • Collaboration & Support:
    • Work closely with the patient care coordinator on scheduling and operational issues.
    • Provide additional support and responsibilities as assigned to meet organizational needs.
