The RoleWe are seeking a highly skilled Patient Navigator to join our team. As a Patient Navigator, you will be responsible for enhancing the overall patient experience by addressing their needs, concerns, and feedback in a timely and empathetic manner.
Key Responsibilities:- Serve as the primary point of contact for patients, addressing their needs, concerns, and feedback in a timely and empathetic manner.
- Serve as the primary point of contact for patients, families, and caregivers, offering guidance throughout the healthcare journey.
- Assist patients in scheduling appointments, accessing medical records, and coordinating follow-up care.
- Educate patients about their diagnosis, treatment options, and available resources to improve health outcomes.
- Facilitate communication between patients and healthcare providers to ensure clear understanding of treatment plans.
- Identify and address barriers to care, such as transportation, financial concerns, and insurance navigation.
We are looking for a highly skilled and experienced Patient Navigator who possesses excellent communication and interpersonal skills, with the ability to engage effectively with diverse stakeholders. The ideal candidate will have a minimum of 3 years of experience in a patient-facing or customer service role, preferably in a healthcare setting.
