
Family Care Navigator

We're looking for dedicated professionals to empower underserved families and take the transformative step of accessing care.
Key Responsibilities:
- Collect, review, and track required documentation from caregivers and families.
- Makes outbound calls and responds to inbound inquiries to guide families through the documentation process.
- Handles a high volume of phone interactions daily while maintaining professionalism, empathy, and efficiency.
- Ensures all documents are accurate, complete, and compliant with state and Medicaid/Medicare regulations.
- Communicates clearly and compassionately with families via phone, text, and email.
- Maintains organized records and tracks expiring documents, sending timely reminders.
- Collaborates with onboarding, credentialing, and family support teams to deliver a smooth experience.
Requirements:
- Extensive experience in customer service, administrative, or healthcare support roles.
- Proven experience handling high-volume inbound and outbound calls in fast-paced environments.
- Excellent organizational skills and strong attention to detail when reviewing documents.
- Clear and professional communication skills, both verbal and written.
- Comfortable using CRM systems, shared drives, or document tracking tools.
- Ability to manage multiple tasks, prioritize follow-ups, and meet deadlines.
- A compassionate and patient approach when working with families and caregivers.
- Familiarity with healthcare documentation, HIPAA, or Medicaid is a bonus but not required.
- Bilingual in Spanish (verbal and written) is a strong plus.
