Role Overview
The role of a Care Coordinator – Registered Nurse is to provide proactive support, education, and follow-up care to patients in the United States. This full-time night shift position is ideal for nurses who are highly organized, fluent in English, and committed to making a difference in patients' lives.
Key Responsibilities- Primary Care Coordination for assigned US-based patients.
- Conduct telephonic outreach and/or in-person assessments based on program guidelines.
- Collaborate with physicians, specialists, and team members to create individualized care plans.
- Educate patients and caregivers on disease management, medications, and lifestyle adherence.
- Track key health indicators and intervene early to prevent ER visits or hospital readmissions.
- Coordinate services across multiple healthcare providers, facilities, and support systems.
- Maintain timely, accurate records in electronic health records or program-specific platforms.
- Report clinical outcomes and escalate high-risk cases as needed.
- Participate in care team meetings and contribute to ongoing planning and improvements.
- Active RN license.
- Experience in chronic care, patient education, or case management.
- Excellent English communication skills both verbal and written.
- Strong clinical judgment and attention to detail.
- Comfortable working independently and using telehealth systems.
- Willing and able to work night shift hours.
- Tech-savvy and familiar with electronic documentation.
