
Care Coordinator

We are seeking a skilled and compassionate professional to join our team in providing exceptional customer support within the healthcare industry.
This is a dynamic and challenging role that requires strong communication skills, attention to detail, and the ability to work effectively in a fast-paced environment.
The successful candidate will play a key role in delivering high-quality care to our members and providers, while also ensuring that our services meet the highest standards of efficiency and productivity.
Key Responsibilities:
- Manage intake of member information, admission/discharge notifications, and requests for services from members and/or providers
- Work collaboratively with hospitals, clinics, facilities, and clinical teams to ensure seamless service delivery
- Process incoming and outgoing referrals, prior authorizations, and census roles with accuracy and attention to detail
- Provide effective triage and care coordination for non-clinical assessment/intervention cases
- Resolve member inquiries and concerns in a timely and professional manner
- Maintain up-to-date knowledge of company policies, procedures, and directives
Requirements:
- HS Diploma or equivalent
- At least 1 year of experience in customer service, analyzing, and resolving customer problems
- Proficiency in Microsoft Word, Excel, and ability to type and talk simultaneously
Preferred Qualifications:
- Experience working in the healthcare industry, specifically with Medicare and Medicaid services
- Background in clerical or administrative support
