Remote Healthcare Support Specialist

We are seeking a skilled and dedicated professional to join our team as a Remote Healthcare Support Specialist.

About the Role:

The Remote Healthcare Support Specialist will provide critical support to our clinical operations, assisting with documentation, compliance, and care coordination to ensure seamless delivery of home healthcare services.

Key Responsibilities:

  • Clinical & Administrative Support
    • Assist in reviewing patient care plans, medication administration records, and physician orders to ensure accuracy and compliance.
    • Follow up with physicians for signed orders and verify proper transcription into patient records.
    • Support the Director of Nursing in tracking missed or overdue nursing visits and coordinate completion.
    • Prepare and verify required documentation for client admissions and discharges.
    • Generate and maintain compliance reports to support audit readiness.
  • Care Coordination
    • Monitor scheduling workflows to ensure PDN visits align with care plans.
    • Serve as a remote liaison between nurses, patients, families, and providers to streamline communication.
    • Track staffing assignments and availability to support the Director of Nursing in resource allocation.
    • Ensure staff credentialing (licenses, CPR, TB, etc.) are tracked and up-to-date.
  • Quality Assurance & Compliance
    • Compile reports and identify documentation gaps to support quality improvement initiatives.
    • Review clinical documentation for accuracy, completeness, and compliance with state/federal regulations.
    • Provide feedback to field nurses on documentation corrections or updates.
  • Training & Education Support
    • Facilitate remote orientation and training sessions for nursing staff.
    • Track completion of required in-services and continuing education.
    • Assist in distributing policy updates, clinical reminders, and compliance alerts to nursing staff.

Qualifications:

  • Current and valid Licensed Practical Nurse (LPN) license or higher (RN preferred).
  • Associate's or Bachelor's degree in Nursing or related healthcare field strongly preferred.
  • 2+ years of clinical experience in home health, PDN, or related nursing environment.
  • Prior experience in administrative or clinical support roles preferred.
