Job Title: Client Care Specialist

We are seeking a skilled and compassionate Client Care Specialist to join our team. The ideal candidate will be responsible for coordinating care and services for clients, ensuring they receive the necessary support and resources throughout their care journey.

  • Coordinate the delivery of services and support based on individual client needs
  • Conduct assessments to determine the necessary resources and services for each client
  • Develop and implement personalized care plans in collaboration with clients and their families
  • Monitor clients' progress and make adjustments to care plans as needed
  • Serve as the main point of contact for clients and their families, addressing questions and concerns
  • Facilitate communication between clients, healthcare providers, and community resources
  • Document and maintain accurate records of client interactions and care plans
  • Participate in case reviews and interdisciplinary team meetings to discuss client progress
  • Stay informed about available community resources and support services
Requirements:
  • Bachelor's degree in Social Work, Nursing, or a related field
  • Minimum of 2 years of experience in case management or coordination within a healthcare or social services setting
  • Strong assessment and analytical skills
  • Excellent communication and interpersonal skills
  • Ability to work collaboratively in a multidisciplinary team environment
  • Knowledge of community resources and support services available to clients
  • Strong organizational and time-management skills
  • Proficiency in using electronic medical records and case management software
  • Compassionate and patient-centered approach to care
  • Ability to handle sensitive information with confidentiality and professionalism
