Caretaker Advocate

The Outpatient Case Manager plays a pivotal role in delivering comprehensive support services across all stages of treatment.

• Acts as the primary point of contact for assigned outpatient clients and their families, ensuring seamless support and enhanced treatment outcomes.
• Coordinates all in-person and virtual therapy appointments, maintaining proactive communication with providers and clients.
• Documents all appointments and ensures accurate calendar management to optimize clinical schedules.
• Monitors client participation in scheduled services and follows up on attendance, engagement, and any missed appointments.
• Clients are assisted with intakes and discharges, ensuring timely completion of required documentation.
• A key aspect is identifying barriers and needs related to housing, education, transportation, healthcare, legal services, and other social determinants of health, and linking clients to relevant resources.
• Maintains ongoing communication and documentation regarding care coordination efforts, referrals, and community partnerships.

• Minimum 2 years' experience in behavioral health, social services, or case management.
• Strong written and verbal communication skills are essential.
• Proficiency in Google Workspace and/or Microsoft Office applications.
• Reliable high-speed internet access and ability to work from a secure remote location.
• Able to obtain a Level One Fingerprint Clearance Card.
